Profound and Multiple Learning Disabilities (PMLD) Care
At SHC we offer a diverse range of personalised care packages for individuals with learning disabilities.
Our services for people with learning disabilities and / or physical disabilities are second to none. Our care is person-centred with the individual at the forefront of all activities and outcomes.
We have specialist facilities, equipment and highly skilled staff to care for a wide range of disabilities and needs. College activities and therapeutic interventions form part of the care plan, and our 24-hour services are purpose-built with aids and adaptations to help the people we support.
What is important to the person?
At SHC we have developed services that support the individuals. Our services for people with profound and multiple learning disabilities are mainly purpose-built with en-suite rooms and track hoisting. Rooms can be personalised in any way that an individual wishes.
Physiotherapy is available for the people we support and there are hydrotherapy facilities available across the group for structured and leisure sessions. We work very closely with schools and colleges to plan good transition programmes for young people moving into our homes, ensuring that the multidisciplinary team are involved, including friends and family. 24-hour care is available within our homes, with nursing support if needed. We offer a wide variety of leisure and social activities and ensure that involvement in the local community is a large part of the individual care plan.
Day care and respite care is also available.
How are services delivered?
There are a number of opportunities available for the people we support to attend colleges of further education, outreach programmes and work experience. These activities can be little or often, depending on individual choice and need.
SHC works closely with local colleges to tailor programmes to individual needs. Some people attend college on a part-time basis, while others attend resource centres.
We also work closely with the Profound Education Team from the Children's Trust who provide an outreach programme for young people who are 18 plus.
SHC also offers residential support for people with learning difficulties within 'ordinary houses, within ordinary streets' in the community. Everyone is encouraged to engage in activities that are meaningful, that they enjoy and that fit-in with their lives. It is also important that individuals are supported to sample activities and experiences - eg: shopping for their own food, mowing the grass, work experience etc.
How does the person communicate?
If the individual is unable to use words, there needs to be clear ways for decisions to be made and articulated.
SHC believes in ensuring that alternative methods of communication are sought via assisted technology, switches, specialist computer systems - touch screens etc. People are encouraged to continue with communication aids and try new ones.
Staff are trained alongside the people we support to use effective communication systems. SHC also retains the services of trusted external speech and language therapists to assist with communication issues.
